Job SummaryServes as the lead for the coordination and project management of Care Gap projects that utilize operational methodologies to drive substantial improvements in quality, efficiency and patient engagement. Provides guidance and direction to the Keystone ACO Partner Organization Clinical Practice Transformation team who will assist with the implementation of action plans developed to improve identified Care Gaps and Quality measure.
