Occ Summary
Provide psychosocial assessments, information, intervention, anddischarge planning to patients and families to assist them in copingwith their hospitalization, illness, diagnosis, and/or lif e situationand meet the continuing needs of patients post hospitalizatio n.
Work Performed
Provide psychosocial assessments of patients and families to identifyemotional, social, and environmental s trengths and problems related totheir diagnosis, illness, treatment, and /orlife situation.Formulate, develop, and implement a plan utilizing app ropriatesocialwork interventions.Collaborate with the health care team a nd involve the patient/family inthe development and implementation of pl ans. Facilitate dischargeplanning across a multidisciplinary care contin uum which includesinteracting with consulting physicians, third party pa yer and otherhealth care providers to facilitate movement of the patient through thesystem and arrangement of an appropriate discharge plan.Provide education and interventions to patients and families to enhanceor re store the capacity for social functioning.Maintain working knowledge ofa nd liaison with community agencies;provide information about and linkage s to appropriate communityservices; participate in area activities rela ted to health and socialservices as appropriate.Under the supervision of a LCSW, assist with screening, identificationand management of victimso f abuse, neglect, domestic violence, rape,etc. Make referrals as appropriate and serves as resource for the healthcare team.May participate inon -call or after-hours coverage.Document findings, plan, and actions taken according to departmentalguidelines and standards.Participate and repre sent social work perspective in multidisciplinaryteams, rounds, etc.Main tain a working knowledge of relevant medical/legal issues thatimpacton p atient care, e.g., advance directives, child and elder abuse.Serve as a patient advocate. Maintain records and statistics inaccordance withdepar tment policies. Attend and participate in staff,committee, department, a nd other administrative meetings.Participate in department and hospital continuous quality improvementefforts.Keep current with social workand h ealth care developments and seek toincrease further enhancement ofjob re lated knowledge.Provide education to other health care professionals.
