DescriptionSummary:As an integral member of the pharmacy team and under the supervision of a pharmacist, is responsible for working with newly admitted patients to obtain and document an accurate medication list to help avoid medication duplications, omissions, and errors. The information obtained will be entered into the electronic medication database system or updated in the database if information already exists. This information will be used by physicians and other health care providers to determine treatment plans and medication orders during and post discharge from the hospital. This position is responsible for providing technical support in all aspects of the delivery of safe, effective, patient focused, and cost-efficient drug therapy.
