ChristianaCare is seeking a Manager of Community Health Operations, specifically over Community Health Workers (CHW), to be responsible for development and attainment of established performance and outcome goals for all CHW programs. The manager will maintain primary responsibility around the operations of the CHW programs within the Community Health Office regarding development and implementation of new programs and initiatives. This position will work to implement strategies with a focus around growth of impactful programs to ensure maximum outcomes coupled with appropriate interventions when need for change is identified.

Responsible for providing the operational direction for CHWs, in the Office of Health Equity, including programmatic operations and developing and implementing new programs.
Designs and facilitates processes intended to advance strategy and objectives within Community Health and the organization.
Supports the Director of Community Health & Social Integration to prioritize and lead projects or major work streams related to Christiana Care strategy, priorities, and special initiatives.
Develops, implements, and overlooks day-to-day operational systems and processes that provide insight into goals, progress, and obstacles for Community Health Workers key initiatives.
Builds and manages relationships with all department heads, external partners, and vendors to make decisions regarding operational activity and strategic goals with leadership from the Director of Community Health & Social Integration
Plans, monitors, and analyzes key metrics for the day-to-day performance of Community Health Workers to ensure efficient and timely completion of tasks.
Create new job descriptions, hire, supervise, develop and evaluate professional staff; develop logical case and rationale for additional FTEs while ensuring maximal productivity under the leadership of the Director of Community Health & Social Integration.
Devises strategies to ensure growth of CHW programs within Community Health and system-wide, identifying and implementing process improvements that will enhance output and minimize costs under the leadership of the Director of Community Health & Social Integration.
Provides oversight and support for CHW staffs that coordinate and supervises day-to-day activities
Conducts regular and periodic staff meetings for CHW leadership to discuss problems, procedures, and new policies and is accountable for the implementation of policies/procedures, reports findings from such meetings to Director of Community Health & Social Integration.
Works with Director of Community Health & Social Integration to develop capital and operating budgets.
Supports managers in coordinating counseling and disciplinary actions; is part of decision-making process as it relates to mentoring and disciplining.
Coordinate resources as needed to support effective IT improvements, marketing strategies, patient experience and practice operations.
Build brand awareness through patient/community experience bringing to life our values of excellence and love as We Serve Together.
Optimally translate key messages to various audiences.
Upholds organization policies and standards. Works closely with human resources to lead team with integrity and establish and maintain a trusting, inclusive and productive environment.
Education and experience requirements:
Bachelor's degree required, master's in management or related field preferred.
At least 5 years related experience with supervisory experience of multiple groups required.
