In 1971, Yale University began a unique and historic experiment in providing health services to its faculty, staff, and students through a multidisciplinary health maintenance organization located on campus. Now, 47 years later, Yale Health now has more than 37,000 members including students, staff, faculty, and their families who come from every state in the country and almost every country in the world. Our state-of-the-art facility at 55 Lock Street, built in 2010, is where our members receive most of their care. It is a 144,000 square-foot medical facility with over 90 exam rooms; a 24/7 Acute Care Department; a 17-bed inpatient facility with two negative pressure rooms; a diagnostic imaging suite including MRI, CT scan, x-ray, and ultrasound; and a full-service retail pharmacy. It houses our 150+ providers, including 111 primary and consulting physicians.

Reporting to the Director of Patient Safety and Quality Improvement, the Project Manager will support the vision and mission of Yale Health to be a premier provider of high-quality clinical services within a safe environment. The Project Manager will collaborate with other project managers, leaders and staff to leverage resources across the organization to advance a robust safety culture through the identification and monitoring of systems that could lead to potential adverse events or harm.
Specific responsibilities for this position include:
Managing critical components of Yale Health’s Patient Safety Evaluation System including, but not limited to: incident reporting and tracking, data collection, analysis and reporting, liaison with ECRI Institute PSO and reactive analyses (e.g. RCA).
Driving risk and error reduction strategies. Identifying actual and potential adverse events and develops systems designed to prevent errors and ensure a safe environment for patients, employees and visitors.
Educating staff in Patient Safety theory and methods to create and sustain a robust, organizational culture of patient safety.
Partnering with members of the Quality and Safety Department, organization leaders and staff to implement strategies to promote a just safety culture.
Required Education and Experience:
Bachelor’s Degree in public health, nursing, or other related field and five years’ related experience or an equivalent combination of education and experience.
Excellent interpersonal and communication skills across all levels of staff including leadership, physicians, administrative and clinical support staff. Positive attitude and ability to make timely decisions. Demonstrated abilities in a healthcare environment, quality management, patient safety or comparable field.
Demonstrated ability leading process improvement projects using standard methodologies such as Lean, Six Sigma, IHI. Demonstrated abilities with data collection, analysis and display including control charts, run charts, etc.
Demonstrated success in presenting complex ideas and projects to individuals at all levels and a variety of disciplines. Demonstrated expertise with safety tools and techniques including adverse event investigation, root cause analysis, systems thinking, team building and training. Knowledge of EPIC or other electronic medical record.
Ability to work independently and collaborate positively and effectively with physician and nursing leadership and across disciplines. Demonstrated ability to lead and coach teams.
Excellent written communication and presentation skills. Independent judgement and decision-making skills. Superior technical skill and facility with Word, Excel and Powerpoint.
Preferred Education and Experience: Experience/familiarity with Joint Commission or other accrediting bodies. Master’s level training in Nursing, Public Health or another health-related field. Experience with business intelligence tools.
